CTSU Intake Form

Please note that during the summer period, July 18 to August 31, CTSU will be operating with reduced services. The intake of new studies and projects is being temporarily paused. We encourage researchers planning to initiate new projects or require comprehensive support in the fall to submit intake forms during the summer.

 

Please note the standard services offered are packaged together and covered by the flat rate fee for industry and external-investigator initiated studies. While these services are charged at the hourly rate for internal-investigator initiated studies, it is strongly encouraged all start-up services are utilized to ensure research adheres to all regulations and are fully compliant. 

Please provide the full study title if available, otherwise, short title will suffice.
Please provide the protocol number if available, otherwise, indicate N/A.

How will coordinators access the subject’s medical record?
What Electronic Medical Records (EMR) system is used?
Who is the site administrator for the EMR?

CTSU Co-Authorship Policy

The International Committee of Medical Journal Editors (ICMJE) defines an individual as an author based on the following four criteria:

  1. Substantial contributions to the conception or design of the work; or the acquisition, analysis, or interpretation of data for the work; AND
  2. Drafting the work or revising it critically for important intellectual content; AND
  3. Final approval of the version to be published; AND
  4. Agreement to be accountable for all aspects of the work in ensuring that questions related to the accuracy or integrity of any part of the work are appropriately investigated and resolved.

Please also refer to the University of Saskatchewan's Responsible Conduct of Research Policy. Our policy is to request inclusion as co-authors on the final publication if warranted based on ICMJE and USask definitions. If CTSU staff have supported the project but do not meet authorship criteria, recognition of contributions in the Acknowledgements section should be considered.
